Help with ADHD meds

Anonymous
My teen is not medicated for ADHD and is really struggling. We're meeting with his psychiatrist on Monday, but I'd love to come with some suggestions. He's currently on Zoloft and latuda. ADHD meds we've tried on the past but had to stop because of side effects include:
Focalin
Metadate
Vyvanse
Cotempla
Azstarys
Guanfacine
Clonidine
Strattera
Qelbree
Wellbutrin

What should we try next? His inattention/lack of focus is interfering with his ability to be age appropriately independent.

What were the side effects?

For the stimulants, dramatically increased anxiety and tics. For the non-stimulants, a variety of problems, including depression (guanfacine), plummeting blood pressure (clonidine), extreme fatigue (qelbree). I can't remember why we stopped strattera and Wellbutrin, I think because we got to the max dose with no benefit.

This is a tough one. Looks like you’ve tried most everything but I don’t see concerta on your list. My child did the best by far on Concerta but only the brand. Unfortunately they can no longer get it approved so the next best has been Vyvanse (I see that was no good). They have significant side effects on Vyvanse including skin picking and tics but they simply can’t function without it so they soldier on. I wish I could afford the Concerta self-pay.

Thanks. 3 of the stimulants we tried were also methylphenidate, like concerta. I wonder if concerta is different enough from the others to be worth trying. We'll ask.

We also haven't tried Adderall. Vyvanse was so awful (the worst of all the stimulants) that I didn't want to try another med in that class, but maybe that is wrong.

My kid did terribly on Vyvanse but great on Adderall

Thank you, that's reassuring!

Honestly, my son had the same thing with increased anxiety with stimulants. Unfortunately, he really needs a stimulant to control his ADHD. The best solution was to continue with a stimulant and add an anti-anxiety medication.

This is really a question for a psychiatrist. If his meds aren’t being managed by a psych, you should consider having one do it. That’s a lot of trial and error for one kid

Meds are being managed by a psychiatrist. My kid is autistic, ADHD, and anxiety and is really, really sensitive to meds.

How old is he? We’ve been through the same thing and now at almost 17 ours is tolerating Azstarys well, she’s been on it a month or so at the lowest dose. I’m not holding my breath but I wonder if it has something to do with maturing a bit too.

18. So we could go back to the ones we tried a while ago.

Already on latuda for mood. We tried Lexapro before Zoloft, but it was a while ago and I don't remember why Zoloft was better. Anxiety is now under control, so I don't want to make changes there. But need to support ADHD symptoms.

We haven't tried most of those, but my kid who had to stop focalin due to side effects is doing really well on Jornay.
