Was this actually an ATC error? I thought it was mainly the helicopter error.

Most accidents result from multiple mistakes/systemic shortcomings, not one.

If the helo was en route from Langley back to Ft. Belvoir, why did they follow the river at all? It's not remotely the most direct path, which would be over land.
