Day 5 blastocysts - concerns?

Hi, may be not worth dithering over, but Columbia Fertility includes in the agreement to transfer Day 5 blastocysts, saying there isn't enough reliable data to demonstrate poorer health outcomes for children born of Day 5 blastocysts compared to Day 3 embryos, but the risks "cannot be dismissed." I'm worried (eggs are being fertilized right now so I literally have 24 hours to make a decision). I see some longitudinal studies (all from Europe that I can see) showing slightly increased risks for Day 5s v. Day 3s of 1) preterm delivery 2) higher birthweights (too high) 3) congenital abnormalities and 4) identical twins. I'm most concerned, as I think most would be with congenital abnormalities. Any (constructive) thoughts/dialogue sought!

FWIW, I know Day 5s have a higher chance of resulting in a live birth - that's not my question here - my concerns are around health of the human born of such a transfer.

I had a Day 5 blastocyst transferred. It was my last embryo. I delivered healthy identical twins. Yes, they were preterm, but twins are often preterm.

My Day 5s are 17 and getting ready to apply to college. They went almost 39 weeks, too. Just one anecdote, but it all went great! Good luck to you.

I only ever made day 6 embryos and have a healthy kid.

I wouldn’t sweat this too much. Focus on success rates. Congenital abnormalities can happen with IVF or non-IVF conception - but are overall still rare.

My clinic would only transfer blastocysts. Wish they had given me the option to transfer D3. My day 6 blastocyst is currently sleeping in my arms. she was delivered at 37 weeks because I had a prior myomectomy but she would have stayed in longer if they would have let her

Thanks everyone! Opting for the day 5 if we end up getting to that stage (7 fertilized eggs as of today....so nerve-racking).

My Day 5 embryo is 6 year old and perfectly healthy.
I was also at a clinic that would only transfer day 5 blasts. I've never heard of Day 3 being preferable.

More “anecdata” to assuage your concerns:

Our day 5’s are currently in 4th grade.

Transferred 2 day 5’s (one hatching, one not). We ended up with twins (not identical).

Yes to preterm delivery (38 weeks, but twins so not too bad). They were on the small side (5 3/4 lbs and 6 lbs). No congenital malformations.

Not sure if you will see this again- but was the 37 week bc of the myomectomy firm? I am 13w, had a myomectomy last year and have been told the same, but am wondering if some OBs will consider pushing to 38 weeks if there are no other risks for early labor. thank!

I was allowed to go to 39 weeks but this was a decade ago. I did have weekly monitoring due to another issue.

My lousy quality 2BB 5 day blastocyst just did an awesome job at his school band performance, and looked so handsome in his suit jacket.
