3 year old cant poop

You're getting a lot of great advice here. I went through a similar issue with my DD. Such a hard time for her and me! She was absolutely miserable holding it in. Ultimately a few weeks of Miralax helped us get over the hump. A few other things we tried:

-DD responded well to pooping on the ground instead of a toilet. I just had her squat down and poop over a plastic bag. It gave her confidence and helped with her fear of pooping.

-I would hold her waist and have her use her stomach muscles and try to push her belly button against my thumbs. It seemed to teach her how she can control her stomach muscles to help get the painful poops out.

-I would pretend poop with Play-doh (Yes, it looked as ridiculous as it sounds). She would laugh and it was a game of trying to push her poop out while I pretended with playdoh.

Other things that I dont think I have seen mentioned:
-poop smoothies
-Book about pooping. This one helped DD understand pooping better. Not a gamechanger but helped make progress:

Wishing you luck OP! I know its tough but I hope it can be resolved quickly.

I'm so glad she's making progress!

You need to completely break the association between pain and pooping. Give up on potty training for now. You need a series of miralax clean outs. I mean like 4-8 capfuls a day for 2 days, repeated weekly. For many kids this totally resolves it. For my daughter at that age we still had to be careful about what she ate, and respond quickly to any sign of constipation but it did work. I do recommend a GI. It’s a common problem.

Lots of good advice here, we went through it was our toddler. Miralax every day to stay regular, and if things are bad and they are crying, don't hesitate to use a glycerin suppository like was linked. It's stressful, but after it is given, they can find relief within 60 seconds and the sun parts through the clouds. Just do it.

He is potty training now too so doesn’t help that he purposely holds poop for 4-5 days.

This is what worked for my kid who tended to be constipated at that age: Full dose of pedialx before bed (but only if he had not had a full dose previously that day!). Next morning drink a cup of warm lemon water before eating breakfast. If warm lemon water is not your kid's thing, a cup of coke or anything else warm/stimulating. Then breakfast as usual. This almost always reaulted in a good dump.

Caffeine does work, but it's hard to dose it for a toddler. Too much can cause heart problems. The amt in a little cup of cola is probably safe-ish. When I was a kid, a full glass of coke was my constipation treatment. Times change.

I used baby prunes and mixed with yogurt. My child liked it and ate it often. Good luck!

Dates if they don't like them chop up and put in something they do. I found they worked better than prunes.

Yogurt like Dannon not greek or kids kind.

My daughter did this as well. I'm sorry OP. This was over 30 years for me and instructions from pediatricians then where awful.

Lots of water.
Plum or pear juice (also kiwi, or dragon fruit).
Begin health prebiotics
ION gut support
Berry fruits - mainly blueberries but others will work too.
My then 2-yr old would scream head off when pooping and within days of starting ION and begin health it was normal poops. Very gassy the first week on begin health but that went away. No more scared to poop, no more trying to hold it in. Now fully potty trained.

Culturelle probiotic + fiber every day helped us a little, after getting things under control with miralax. Although it's still not perfect...

When it was really bad, agree just do whatever it takes.. diaper.. pretend to be a dog..
