I understand that I hold myself to the highest standard possible, which may not be practical for most parents, or nannies, for that matter.

More than anything, I hope that every parent can learn something from the unbelievable tragic fate of these two poor children. If you are not seeing "eye-to-eye" with the nanny, make sure you work it out. If not, TERMINATE. Immediately. Either that, or refine your wishes. But, never, NEVER leave your child with a person who is behaving "off". Learn to trust your gut. Nothing is more important than your child's basic safety. Nothing. Ever. Not even your "second income" that you think your family can't live without.
