Hopefully reasonable Mb here: call tomorrow and say you don't feel good enough to come in on Monday. My husband and I are in the same boat as your bosses (demanding jobs, no family in town, but we give our nanny sick days and we wouldn't fire her for using them. Yes, it sucks when our nanny can't come in, but we figure it out. You were in an accident and are in pain - stay at home and rest, and use the sick days. Your employers understand that life happens, and if they are decent people, they won't begrudge a day or two off after you've been in a bad accident.
I agree with this, but I would be annoyed if you didn't make the decision until Monday morning because you didn't want to use up your PTO. Call them ASAP and tell them you can't work Monday per doctor's orders, and see what they would like to do going forward.
