How to go about contributing to nanny’s health insurance premium? Can we just pay cash or is there a right way of doing this and it not getting taxed.

To answer your questions: Different employers do different things.

Some do nothing, others will pay all of the very best policy. Most fall into nothing or very little; better nannies expect at least 50% (unless they’re covered by their spouse’s insurance). Method of payment varies from a special payment to nanny monthly, nanny running household card or payment sent directly to the insurance company.

My family write me a separate check once a month to cover my insurance. They write in the memo line " health insurance" and its not taxable.

A stipend for health insurance is taxable income. You may want to try a QSEHRA which is tax-free for contributions and deductions.
