MBs: How do you conduct annual performance review?

| We have an annual written performance review for our nanny in our contract. MBs who've done this before, I'd love to hear your experience. Do you fill out a form and ask nanny to sign? Do you ask your nanny to do a self-assessment? |

| Self assessment plus assessment on us. Her self assessment is pretty casual though. More just a way to address any things she's feeling insecure about in her job (last year she brought up how she thought she wasn't being outgoing enough in order for 2 year old DS to make friends). |
