Call. Right now. Say you have found the physical demands of the job to be far more taxing than you expected and you cannot continue. It would be more professional to offer to come in for a few more days if needed, but with whatever boundaries you need (no wearing the baby, rocking him in a glider but not carrying around, etc). They can choose to take that or leave it.
