If the child is able to move their head away from anything soft, and roll away if turning the head doesn't work, they are old enough for a blanket, stuffed animal or other options. On the other hand, you specifically told the nanny not to do something and she's still doing it. If this is a huge dealbreaker for you, fire her. If it's something that you can live with, talk to her again and let it go if she keeps doing it.

I'm sorry but this is a situation that will not work out. If she won't listen and follow your directions on the most basic, most important things, then it is only going to go downhill from here.
