It depends on the situation. If the family only needs care during normal hours that a live-out can do, and they offer room and board as an option to make it more enticing for the nanny, legally they can deduct rent. If the family needs crazy hours, they need someone overnight frequently, they need on call hours that simply aren't possible for a live-out nanny, no, they can't deduct room and board from salary.
