I guess it also comes down to how did she clean it? You mention rinsing it off in the sink, while others have mentioned she could have boiled it in water to help sterilize it. I wouldn't be happy about a nanny that just rinsed it off in the sink under running water. If it was boiled, then that is a different story and is acceptable for use even if you choose to get another toothbrush as a replacement.
I know I wouldn't want to keep using one that I had dropped into the toilet as I would keep thinking about that each time I used it, even after sterilizing it. It could be used until I got a new one if I didn't want to go for a day without a toothbrush though.
This is one of the reasons that I keep the lid to my toilet closed, no accidentally dropping things into it (or having my cats fall in, hah). You also lessen the chance of germs flying in the air from the toilet flushing if you close the lid first.
