At the 1st follow up, he sent my husband for a follow up semen analysis and that we had to pay out of pocket for ($189).

I have Aetna with DC Government. The plan says it covers all diagnostics for infertility. Then it goes to 50% coinsurance for IUI, which is at the negotiated rate, so it's $187 I pay per cycle. I have no coverage for infertility drugs, so those are all out of pocket. We normally do $500 in our FSA so I upped it to $1,000 this year. We have no IVF coverage, but aren't sure we'd do it anyway. I did have to enroll in an infertility program with insurance, which was free, but took about 45 minutes on the phone to enroll. I think it's just making sure we've tried everything first. A nurse then called to follow up and was actually helpful going through the procedures AND what they will pay for.
