Kid didn't double-check the forms to see that they're signed before turning in.  Even my kids are able to do that.

You expect a 3rd grader to 'check the forms' to see if parent missed one signature out of 20? My 3rd grader is in a private school also and we got a giant packet of stuff to fill out, but my school does not do inappropriate punishments like taking away a child's recess for that, and would simply send it home for parent to sign.
