I apologize for my error and the inconvenience for you, as upon checking I've realized that I failed to register you for this year's conference.

I have contacted the conference organizers to arrange for your immediate registration and will be back in touch asap to provide an update.  If you are handling it yourself, please let me know.  If you need to get in touch immediately or prefer texting, my cell phone is XXX XXX XXXX.

Going forward I will implement better systems to ensure I do not repeat this mistake.
