Per camp.[/quote] It’s not per camp. We have our Dr fill out a blank Virginia School Health form at the annual well visit. It’s free when we bring it with us, and $35 if we need to request it special after the fact. We fill out everything except the name of the school / camp at the top of the form. We scan it when we get home and then print it out and fill in the camp name as needed. [/quote] Ever camp we use has a different form. Are you saying all your camps use 1 form?[/quote] Yes. In Virginia all schools, preschools, daycares and camps use the exact same form from the State of Virginia. Some camps may have additional forms and disclosures, but the medical form that is signed by a physician is standardized. [/quote]
