No and the thought never crossed my mind for camps or school. DC has two doctors (ADHD meds and pedi) worth of forms - all of their doctors have patient portals. I download the PDF's from website, upload them with a message to relevant doctor, they usually come back within a week or two but I send them as soon as I get them so I know have a few months to get it done if they are slow. I've never had a camp or school request dentist signature, only contact information.

i can afford the physical. Each form is 35$. Each, per child. Per camp.


I just get one form per kid per physical and send a pdf of the same form to camp, school, etc.

Absolutely not. Do you not have a few minutes to send a form to your doctor to have it completed?? Not worth all the downsides to forge someone’s signature on a medical form.

It’s not per camp. We have our Dr fill out a blank Virginia School Health form at the annual well visit. It’s free when we bring it with us, and $35 if we need to request it special after the fact.

We fill out everything except the name of the school / camp at the top of the form. We scan it when we get home and then print it out and fill in the camp name as needed.

Yes. In Virginia all schools, preschools, daycares and camps use the exact same form from the State of Virginia. Some camps may have additional forms and disclosures, but the medical form that is signed by a physician is standardized.


Well that's one thing Virginia does right. Nice to know.

OP here. I'd argue it's about the same thing. Also, I do have a doctor friend...not sure I have the courage to ask though.


If you can fill everything out for your friend and just bring/email the forms for a signature and stamp, maybe. A gift might be unethical. A lawyer friend once did us a big favor, but said he was uncomfortable with our (small!) gift, because he didn't want a quid pro quo situation. But express your gratitude in some way.

My husband is a doctor, but from another country, and does not have a license to practice in the US, so it's even more irritating that I need to go through conventional channels when I have an MD/PhD sitting right next to me!

I would never do this.

But I've never needed a medical form for any camp, and I'm coming up on my 3rd year of camps for DD. She doesn't do sports camps--is that the difference?

We have a friend in our neighborhood who is a PA and in a pinch DH buys him a six pack and he comes over and does a cursory check and signs the forms.

My kid goes to Camp Sonshine every year. I check the box that says she goes to mcps and no forms are needed. Been going for 5 years.
Art camp doesn’t ask for signed medical forms either.
