What kind of unlicensed, uninsured camps do your children go to that don’t require forms? What state do you live in?
Anonymous wrote:Anonymous wrote:No? Why would you want to do it? If you’re in DC and can’t afford a physical, there are places to get one for very low cost (maybe even free?)
i can afford the physical. Each form is 35$. Each, per child. Per camp.

It’s not per camp. We have our Dr fill out a blank Virginia School Health form at the annual well visit. It’s free when we bring it with us, and $35 if we need to request it special after the fact.
We fill out everything except the name of the school / camp at the top of the form. We scan it when we get home and then print it out and fill in the camp name as needed.
Ever camp we use has a different form. Are you saying all your camps use 1 form?
Yes. In Virginia all schools, preschools, daycares and camps use the exact same form from the State of Virginia. Some camps may have additional forms and disclosures, but the medical form that is signed by a physician is standardized.

OP here. I'd argue it's about the same thing. Also, I do have a doctor friend...not sure I have the courage to ask though.

same poster. I would be tempted though OP if I had to pay a lot in fees. It’s gotten ridiculous.Anonymous wrote:My kid goes to Camp Sonshine every year. I check the box that says she goes to mcps and no forms are needed. Been going for 5 years.
Art camp doesn’t ask for signed medical forms either.
