Prior to deinstitutionalization, they were much more rare. There will always be mentally unstable, violent people - they have existed since the beginning of time. Bringing back mental institutions might help some, but people cannot be forced to accept treatment - witness the large numbers of mentally ill homeless who refuse shelter and treatment.
