It was issued by the New York State Department of vital records. It had my full name, it had the full name of my mother and the full name of my father. It had the date I was born and the location of the birth, and it was signed by the correct authority. It has the embossed seal of the office of vital records. It was not a hospital issued keepsake. It was the birth certificate issued by the state of NY. But because it was the word registration on it I think it confused people from other states. This wasn’t the document issued by the hospital to prove a birth certificate has been requested. This was the actual certificate that the office of vital statistics sent to you as your birth certificate. I used it for my passport![/quote]
