This document IS a birth certificate. It is not a keepsake issued from a hospital. It is an official certificate of birth because it has the following:
- issued from the Department of Health of New York State (the correct agency)
- full name
- full date of birth
- location of birth
- parents' full names
- signed by the relevant official
- With an embossed/raised seal from the relevant office.
The problem with it is they use the word "registration of birth" on it. That confuses officials. But it is the official birth certificate for New York State at the time.

You apparently work at DMV as well. That document appears to be a signed government document complete with seal. Not clear how to make it more authentic. It provides the exact same information that a "birth certificate" does. You can't get your original birth certificate. Only copies that certify that there is an original on file somewhere.
No, just volunteer for my children's soccer league in MD, and we have to abide by the rules of MSYSA. They say Birth Certificates, not Certificate of Birth. Not my rule...
