This still means that BOTH pilots got confused/mistaken about which plane they were giving "visual separation" to. Hopefully one day we will know why. [/quote]

Guy in the back does visuals. Pilots are looking at controls and gauges. They literally say: going inside (to do something); going outside (to look) [/quote]
