That being said, the psychologist was extremely confident in the score, described the situation accurately in the report, and DD was accepted to AAP (keep in mind that since there is no official diagnosis I couldn't include any reference to the possibility of ADHD in the application).  The statement that the scores were 90s vs. 30s it what really makes me think there is something strange (other than ADHD) at play here; based on what you've said, I'd be more inclined to believe there was a problem with the way the test was administered.  Despite DD's extraordinary fidgeting, question asking, and various other attempts to "self-sabotage," all scores ranged from the 94th-99.7th percentile.
