If someone had to have a hysterectomy when they didn't want one (let's say due to a medical problem), that is sad and you offer emotional support. When I CHOOSE to go into the doctor and get a hysterectomy because I am tired of dealing with my gynecological problems, I won't be weeping, I will be relieved. And finally, who says we don't offer condolences when people have abortions? Most people can't really talk about having an abortion cuz you know, judgy mc judger and all that. But if I had a good friend who TOLD me about her abortion, of course I would tell her how sorry I was. It's not something a person does lightly or for fun.
