I think Tess and Randall are dreading the visit because they're going to pull life support from Rebecca, or maybe she just had a massive heart attack or stroke, and that's why they say "I'm not ready." Theory Two is darker -- it's Annie, and she's gotten into trouble with the law or had an overdose or something like that. That explains why Randall and Tess aren't ready to see her, and maybe the Toby comment is simply that the entire family is gathering and when he says "she doesn't want to see me" he means Kate as they're now estranged (but doesn't quite make sense why he would be part of some kind of family gathering).
