My kids have the same:

First name/middle name/my surname as another middle name/father's surname

The only hassle is when a form doesn't give much room for middle name.

Having my surname in there is handy for travel, because I did not change my surname when I married, and I often travelled overseas with them without hubby. (Still, I usually carried a notarized authorized from my husband).

We did this too!  My son is only two, but so far no real issues.  Planning to do the same with #2 due in March.

My youngest sister has two middle names (she's in her 20's) so I consulted her, and her names are really long and that's been the only issue - they don't fit on most standardized forms.  So we chose a super short (4 letter) middle name for my son.  [/quote]
