Similar question as OP, just to clarify- if my kid's last physical was in September and the Fall sport he wants to try out for has the season ending in November, is that still OK? What about if he's also interested in a Winter or Spring sport? In other words, is it valid throughout the whole year, or does it "expire" exactly 13 months later?
I there a reason your child doesn't get an annual physical?
Ummm, he does? His birthday is actually in the summer, but the ped's office keeps wanting to schedule it at least a year after the previous one, so this is where we're at. So if I went to them and asked them for a physical now, they'd probably say no (can do a quick one at the Urgent Care, but I don't want to unless it's necessary.) It seems like I can just submit it now and then have them redo the form after his next physical, to keep him current.
