[/quote] If your child's physical was last fall, you can ask their pediatrician to complete the pre-participation physical evaluation form (I just dropped it off, they filled it out, and I picked it up and submitted to school). Once you've submitted that form you should be good to go for 13 months. Hope that helps.[/quote] It’s 13 months from the date of the physical not the date the form was filled out. And you have to have a new one on file as soon as the old one expires or your kid can be pulled from sports. Our AD is a stickler and you get a notice that your kid will be pulled if you don’t get it updated. Honestly if your kid is trying out for a Fall sport, the easiest thing to do is have it done in late July or early August, depending on how fast your doctor is at doing the forms. Also you probably know this but in case you don’t, tryouts for fall sports are the first full week in August and some sports have multi day tryouts. [/quote]
