[/quote] If your child's physical was last fall, you can ask their pediatrician to complete the pre-participation physical evaluation form (I just dropped it off, they filled it out, and I picked it up and submitted to school). Once you've submitted that form you should be good to go for 13 months. Hope that helps.[/quote]
