The answer is bedding companies are making linens way too big on purpose, because it's cheaper to have linens that are too baggy than ones that are too small (and subsequently have people return them). Hence we all are sleeping on terribly rumpled bottom sheets.

A good duvet cover should be the same size or smaller than the duvet insert. This keeps the duvet nice and puffy. You should not need ties to keep it in place, that's unnecessary if the duvet fits nice and tight. You shouldn't even need buttons on the duvet cover, frankly.

To wit: I recently purchased two double duvets for my king size bed. Do you know how hard it is to find a duvet cover in the double size? The stores only sell twin size, then double/queen, then king. Well. I tried the double/queen and my duvet flattened like a pancake. Heck I tried my old queen-sized comforter in the cover and it was absolutely swimming in it, too. Just horrid! I ended up going with a twin duvet cover for my double-bed-sized duvets, which worked great and I sleep under a puffy cloud every night. But I literally had to outsmart Big Linen to do so. grrrrrr.
