My swimmer is a college senior, and I sure we don't still have it-

The D3 schools my swimmer was considering, academics were important, so the letter included, grades, classes, intended major and test scores and other interests.  I remember it included a paragraph about their swimming and times (definitely if they would score at conference meet) and information about their current training (club and HS) and any awards/cuts.  He also completed the online questionnaires for each of the schools.  Some schools never responded, and some took a while for them to get back to him.

I am sure an older teammate, club/HS coach or even AI can help with samples. What we were told, is if you are realistic and have a large number of choices, you will find a place to study and swim.
