In a pinch all you need to do is put the clean diaper under the child prior to taking off the old diaper so that their skin isn't in contact with the changing table.

Sorry but no way am I putting my child on a changing table without a pad, clean diaper or not. i know germs are good for them but I have witnessed far too much crap (no pun intended) happening on them (errant pees, blowouts, etc) that I'd prefer not to have my child on it. But then I'm the type that won't sit on a public toilet even with a seat cover on it.
