Ped didn't finish DC Health Form

We got a last minute offer to go to a daycare in DC near my work instead of our planned VA location. So we had a last minute scramble to get the DC Health form filled out. We called, messaged, everything but didn't hear back and finally DH went to sit in the office with the form. They took it and said they would do their best.

Peds office just emailed us a copy of a "Visit Summary" which shows DC's immunizations and birthday, but nothing else. Not even height or weight. It is missing the TB and lead exposure information.

We are supposed to take DC tomorrow morning for first day of daycare. Will the immunization record suffice for now to get DC started or will we need to keep her home until the Ped fills this form out?

In my experience, TB and Lead are required for attendance.

Has your child had these tests done? I know they are not required for all local areas so they may be additional tests that need to be done.

Lead is a blood test, TB requires 2 visits.

Can you go to an urgent care to get the rest of it filled in? I don’t know the DC form, but in other places for TB we didn’t have to actually be tested for it, just screened for exposure risk. The doctor just had to sign that we were considered low risk so no test was needed.

Go to Urgent Care or CVS Minute Clinic. Then get a different pediatrician.

Our excellent pediatrician practice takes 3-4 days to complete the health form. OP had a last-minute request. If they are otherwise good this is not a reason to change.

Can you just fill it out yourself? It’s not like they’re going to check.

Thank you for the advice. We called PM Pediatrics and they are so nice but could not help us. CVS does not do the lead levels. We have everything from DC's 18 month checkup, full report so submitted that hoping it works. We are calling daycare first thing in the morning and if they say DC can't come then we are going to go for a walk-in visit at the Ped.

Yes we were late on this one but it has to do with switching from VA to DC. I am considering switching because I can't ever get anyone on the phone. Love the actual Ped but the office is overwhelmed.

UGH! DC's first experience at "school" and we have done so much prep, DC already asking to go every morning.

All you need is the signature/stamp, OP, and then you can fill out the info yourself.

Yeah they would need to actually respond to us to do this. But hopefully we can press them tomorrow morning in a walk-in.
