Ultrasound and sonohistogram don’t agree

IVF pregnancy resulted in a c section 14 months ago. Came back to my clinic to be evaluated for another transfer, the RE said we should do a sonohistogram because of the c section, which I was happy to hear. I peaked at the ultrasound result before the RE came in for the sonohisto and saw the tech recorded a 7mm defect. The sonohisto showed a 1mm defect and the RE raved about my “beautiful” uterus, said I can try a VBAC. I chose to trust him because the sonohistogram is supposed to be more accurate. Today at the embryo transfer, as he is literally squirting the embryo in, he says “there’s an isthmocele, I recommend a c section”. I can see a black bubble at the c section scar and that the uterine wall is not smooth and continuous. I objected and said the sonohisto was good, he flips over to the record and doesn't really say anything, I point to the spot on the ultrasound and ask "What is that, scar tissue" and he says "You can't really see anything that way," or something like that.

So, what result do I trust? Do I get a radiologist to look at the ultrasounds? Anybody have this happen and how did it turn out? I am worried we wasted a good embryo on a crappy uterus.

Forget the RE- bring your file and talk to your trusted OB. It’s their call anyway.

*That is in terms of vbac or C section. For the transfer, just see what happens. Fingers crossed and then take it from there.

Well I have no choice but to do that unless I stop taking my meds because he injected the embryo into my crappy uterus without my consent.

The fluid sitting in the defect lowers my chances.

The first baby was delivered by a midwife so I have no established relationship with an OB. I actually tried to follow up with that OB but she flat out refused.

Transferred to the care of the OB for a few hours. I wouldn't go to her anyway now because she might take it as a personal attack on her skill as a surgeon and that is drama I don't need.

I would ask the midwife for an mfm recommendation. Do you want to share your location? We might have some collective recs.

Oh, the midwife picked my induction date because this OB was working at the hosp so, that may not go over well with her either. :/ Probably better to go with somebody totally new.

Are ultrasound images part of the medical file or do I need to have them repeated if I have a BFN or what?

I think there is some unnecessary drama here. No OB takes it personally that there is a c-section related scar or complication. No embryo is "injected" without consent.

Call or email our RE and ask if he or she would please let you know the specific concerns since you've heard different information at different points. Take it from there. Do this if you do or don't get pregnant this cycle (which likely has more to do with factors unrelated to your section scar than related to it.)

He 100% transferred the embryo without my consent. I walked into a VBAC potential situation (what he said during the sonohistogram) and told me about the isthmocele as he was inserting the catheter and before I could say yes or no to repeat c section he injected the embryo. It happened within seconds. I tried asking him about the discrepancy while I was still on the table and he didn't feel the need to clarify so I pretty much left in anger. I have had many problems with this doctor. He is the director of the clinic and thinks he's allowed to do whatever he wants.

The c section defect was already on my radar because I know another woman who had one she needed corrected because it was causing her secondary infertility and when I was still post partum I contacted the OB's office trying to ask her about that. Her assistant told me to ask the midwife. So I am 100% done with that OB.

Now if anybody would like to stay on topic, that would be great!

+1. Also is OP saying they would have canceled the embryo transfer because of a potential repeatbc section? Because that's weird. No one can guarantee you a VBAC (or nit having a c section in the first place). You were there for the procedure but you expected the doctor would know you would want to suddenly cancel.

You have got to reframe your thinking here. You're making everything more dramatic.

You people are terrible. The c section scar lowers chance of implantation. No, I absolutely did not want to waste a good embryo on a crappy uterus. I would rather donate them. Or I could look into have the c section scar reduced. Both of my prior placentas were low and anterior. I have two kids, I don't need to have a placenta accreta bedrest situation and/or be looking at terminating a wanted pregnancy. I don't expect the doctor to "know" anything, that's why he needs to ask for CONSENT. Ever hear of it? I consented to an attempt at a VBAC, not an automatic repeat C section.

Go see Dr. Lempel at INOVA. He's a phenomenal OB, believes so strongly in non-intervention that he instituted new policies that lowered their c section rate by 10%. He practices evidence-based medicine (I have a PhD and have fact checked some of the very specific instructions that he gives). He will give you a good recommendation one way or another.
