My teen received his first Pfizer shot today at CVS. I got an email confirming that he had received his first dose, but for some reason, the last name is missing the second part (he has a hyphenated last name), and the address is our previous address, which we left ten years ago! The PCP and their address are also incorrect - it's someone we've never seen in Baltimore, a city we've never lived in. The first name, gender and birthdate are the only correct data.
So. How do I go changing all of that? I am concerned that this incorrect information will create issues during future travels with a vaccine passport. |
That’s weird. How did they get the old information? You should call CVS to straighten that out. |
I would reach out to the pediatrician to see what they advise.
