RE for this scenario

Hi all, I’m 36 with unexplained infertility. Done two rounds of IVF at SG with Dr. Lauren Roth.
First cycle, 26 eggs, 20 mature, 13 fertilized, 4 blasts, only one normal, transfer failed.
Second cycle, 13 eggs, 9 mature, 7 fertilized, 3 embryos, two abnormal one mosaic.
Both cycles were agonist with lupron trigger.
Not feeling like RE is very creative with what to do next. Wants to go for a long protocol to “just try something different”. Anyone have any thoughts about what to change up or who might be a good doc to see in the area? By no means committed to SGF.

I haven’t really seen any great options for your problem—not making normal embryos. In general, the belief seems to be that protocol won’t impact hpt-a status, just blastocyst rates. Have you looked at it starts with the egg? Some antioxidants in there may help embryo quality. Melatonin in particular is probably worth trying. Other options would be to use HGH, but there are no studies to show it helps and I wasn’t able to get SGF to prescribe it. You could also consider not testing, which I know is scary, but PGT-A is not perfect and you could actually be discarding embryos that would result in a live birth.

Did you have high doses of stims, OP? My worst cycles were the ones where they really jacked up the meds.
My RE at SG advised taking ubiquinol (Coq10) to improve egg quality.

How’s sperm quality? I’d check DNA fragmentation at this point too, just to be thorough.

OP here. Thank you for all of the recommendations. I will certainly be looking into them. If anyone has any more thoughts on good REs let me know. In an ideal world, I would like to do PGA testing, and use that to rank order which embryos I implant. I have been really frustrated with Shady Grove for not letting me implant my low mosaic embryo.

During my first cycle, I think I was overstimulated. The embryologist said that my 20 mature eggs were “squishy“. I had lower stimulation during my second retrieval, and the embryologist said the eggs looked better, but still no normals.

Would you consider going to Cornell? It would be tricky to go during covid-19 but they are great with tough cases.

You might see if another clinic will transfer your mosaic embryo. I was at Dominion and was told they would transfer a mosaic after all my normals were used. I am shocked SG won’t do that!

Do you have PCOS by any chance? I didn’t actually realize I did until I started seeing an RE. I went to CO for CCRM there and had great success. They put me on supplements and did completely different stims. It didn’t actually involve that many trips there and was so worth it. Good luck
