Specific provider recommendation for a D&C

I just found out at my 12 week scan that my baby no longer has a heartbeat. Because he is measuring over 11 weeks, my doctor says I need a d&c. He wouldn’t even give me the option of pills. I’m totally freaked out about the possibility of scarring because I feel like I’ve already got the deck stacked against me—this was a PGT-A normal embryo that was hard fought for and I also have had one other miscarriage of an untested embryo at 6 weeks and no living children. Because of the holidays GW can’t get me in for a d&c for a little while (hopefully midweek next week), but I’m wondering if anyone has specific doctor or clinic recommendations for d&cs. Also should I request an endometrial biopsy to check for infection and a hysteroscopy to check for scaring before doing another transfer? We only have two more PGT-a normal embryos and are hoping for 2 children (I’ll probably do another retrieval before transferring too).

I’m sorry... but why can’t your regular doc do the D&C? I assume it is an OB-GYN.

I had six miscarriages along the way....I’m sorry for your loss.

I'm so sorry to hear this. It seems like your OB should be guiding you through this process. Mine at Capital Women's Care was great and everyone at Sibley took good care of me. Wishing you all the best with your remaining embryos. Another retrieval sounds like something to consider.

OP, your regular OB/GYN should be fine to do this. If you don’t have a regular person, make sure whoever you go to understands that you are actively trying to get pregnant so they will be mindful of scarring. I had my d&c at Georgetown and pretty sure a resident or med student performed the procedure. I got pregnant again a few months later. Good luck and sorry for your losses.

I had a D&c for a missed miscarriage at the same point where you are, and I got pregnant with my son six weeks later. Why isn’t your doctor doong the d&c? Many OBs won’t do abortions but they should do a D&c or have a colleague to refer you to.

Thanks everyone. Yeah I was going to an MFM and they don’t do D&Cs. They did kind of refer to me to a colleague I think, but because of the holiday everything is all screwed up. I got diagnosed with the MMC on Wednesday morning and no one has contacted me yet about scheduling. I’ve left messages with my MFM but haven’t heard back. He said the name of the doctor who would do it but I was too stunned to process or remember the name. I just want to get it scheduled so I can move on.

covid has definitely affected the d&c scheduling at gw. I had a mmc earlier this year and it was going to take them about a week to fit me in. that said, the best d&c experience I have had was an in-office (no general anesthesia) d&c at gw with dr. jamshidi. very sympathetic and caring, and they were on top of it with the meds.

i had an in-hospital one at georgetown and you have to go to labor and delivery for the surgery, which is just awful. the doctor that prescribed the tylenol 3 wasn't authorized for opioids so i was stuck with just advil afterwards. also i got a $1800 bill for anesthesia like 6 months later when i had already switched employers and couldn't contest the denial effectively.

you may also want to check with your RE to see if any of them would do the d&c, otherwise I would wait for GW.

my RE had me do a saline sono to check for scarring before starting another FET.

Also- I'm so sorry for your loss. I'm at 10w and having some bleeding with my last PGT-normal DE, and expect that my next scan will give me similar news. It's heartbreaking.

Her baby is measuring 11 weeks though. That’s a big differnce from you MMC which stopped growing at 6 weeks but wasn’t discovered until later. I’m so sorry OP. You could also wait until it happens naturally if you are up for that. I would get pain meds from your doctor. That will minimize a chance of scarring.

I have no idea if they would do it for you, but I had my D&C with Dr Malik at Shady Grove Fertility in Rockville. I have been pregnant three times since (1 other mc and 2 live births) so I feel like it was successful and no scarring or anything.
But I was already a patient with SGF so I was able to easily schedule. Not sure if they take "walk ins" so to speak.
