Egg retrieval billing at Shady Grove for Carefirst patients

Hi I am looking over my billing statement with SG and they billed carefirst $1500 for charge - Egg Retrieval ASC. I am assuming that is their facility charge and my question for those that have carefirst (I have the gold 1750 option through the MD exchange),how much did the insurance cover? I have heard some say $300 and some say they weren't charge. I received an EOB saying they didn't cover any (no reason why) that I was responsible for half. This doesnt seem right. I think they may have charge as a hospital facility but isnt this considered a non-hospital facility?

Anybody have any feedback on this charge? Thx

Did SGF submit an authorization? Was it approved?

I have the same plan. An egg retrieval should be:
$30 for the anesthesia
Co-insurance for the insemination (it depends on how many eggs there are)
The full facility fee (about $800 is the allowed amount) until you reach your deductible, and then $75
Doctor charge should be covered 100%

If they're saying your claim for the facility fee is denied, then the only thing I can think of is you didn't have pre-authorization for the procedure.
