Any ladies with DOR or POI that can recommend a good RE? I am 41 and really hoping to have another baby. I heard that it really comes down to a doctor that can “prescribe” the right protocol and understands these issues. Thank you.

| Unfortunately, none of the clinics in this area are good for AMA/DOR. You’ll have to travel out of state to have a reasonable chance. Consider trying Dr Davis at Cornell. You can consult over Zoom. I was able to get in fairly quickly last month. He does want all of your records before the consult, including any images from diagnostics such as HSG on a CD, which can take some time to get. I did my baseline ultrasound and blood work locally and then went up to NYC for the rest of the cycle. I probably could have done the next 2 visits locally, but they encourage monitoring there. Most clinics here will give high dose stims, then push donor egg for AMA/DOR or tell you there’s not much they can do for you when it doesn’t work. Dr Davis uses clomid or letrozole with lower dose stims which may work better for DOR. |

I am DOR and a poor responder and did Cornell protocols at GW. We tried one cycle on high stims, then tried Lupron flare, then switched to Letrozole and mini stim cycles. Dominion also does mini and natural cycle IVF. If I was just starting IVF, I'd probably do a consult at Cornell and the local clinics, then decide. |

| I started at GW and they did low dose protocols there. Use Dr. Frankfurter. Low dose antagonist worked for me. |
| POF is very hard to overcome. Research before you start spending. |
Beware of GW and Dr Frankfurter. GW does not do weekend monitoring. My cycle day 6 of stims was on a Friday. They told me to trigger on Sunday without any monitoring visits between that Friday and the Sunday trigger. It was my worst cycle ever and only got one day 3 embryo. Dr Frankfurter said the bad results was because of DOR and there was nothing they could do for me. I’m 39, AMH of 0.9, and FSH of 9. I know my numbers aren’t great, but they’re not that bad either. Other consults after this thought my cycle at GW was mistimed and couldn’t believe GW didn’t monitor the days leading up to the trigger. Dr F was really pissed when I questioned the cycle timing, and I pointed out I had objectively better results elsewhere when monitoring was done daily. |
| I did an estrogen priming protocol. I had to change doctors as the first one always chose protocols that started with birth control pills, which over-surpressed me and led to 1-3 eggs at retrieval. |

New poster here - I'm in a similar boat as OP. I'm 41 and have done 5 IVF rounds at one clinic here in DC ("medium dose") and then switched to SGF where I was on a high dose protocol. The high dose was my worse cycle, but I don't think SGF does low dose protocols, so I'm thinking about where to go next. How long were you in NYC? Did you rent a hotel or an airbnb? I'm trying to figure out the logistics with covid. (Please no judgement about how many rounds we've done - I'm generally a good responder and we really want at least 1 baby). |
I was in NYC for about 2 weeks. I did my baseline monitoring at Shady Grove Rockville. However, they screwed it up royally and sent my info the wrong clinic. The worst thing is that it is impossible to speak to a human at Shady Grove and so I ended up playing phone tag between Cornell and Shady Grove answering service. I did the rest of my monitoring in NYC after this, although technically I could have stayed locally for a bit. There is a point Cornell wants you in NYC, although I don't know what that is. I did an airbnb for most of the time. When it ran out, I went to Helmsley Medical Tower (affiliated with Cornell) for the rest. For COVID, they want you to send a COVID test before they will start your stims. It tricky because you can't send it in too early, but you have to make sure it returns quickly. I did the LabCorp pixel test, which returned quickly. They tested me again for COVID just before retrieval. You may have to quarantine in NYC depending on the COVID rates locally at the time of your cycle, but you are allowed to go to medical appointments. My Cornell cycle was not successful. I just had a follow-up with Dr. Davis. He said it's not my age (I'm late 39) and has a plan for the next cycle. I'm debating whether to try another cycle with Cornell or give up. I don't think my options are good locally. |
