I'm always engorged and my boobs hurt

I am EBF my 9 month old. She is STTN. I wake up massively engorged and my boobs HURT and my clothes are soaked. Long story short, I am probably making exactly enough for her because I EBF and never pumped between 3 months - 6 months when the nanny started. But my boobs still always hurt and I feel like I can never empty them.

So how many hours do you go between feedings?

Do you pump before bed? That’s the only way I survive with my 6 month old who’s starting to STTN more reliably.

No I wanted to get my body to regulate and get rid of my oversupply. I had so much in my freezer I donated it all to a milk bank.

She eats at 730, 11, 2:30, 5 and 7. Naps are 930 and 330 and bedtime is 730.

Oh man. But you said you think you make exactly enough? Are you looking for tips for surviving the night without getting painfully engorged?

Same PP. I’d skip the last feed, use some frozen milk instead then pump in place of the last feed (closer to your bedtime) so you can get some rest.

Oh I'm sleeping 11-730. It's glorious. I used to wake up from the boob pain but it's not THAT bad anymore. I don't have any frozen milk. I donated it all a few months ago (almost 200 oz).

I mean, right now for example, I pumped at 2 to give the nanny milk. I pumped for 20 minutes to get enough for a 4 oz bottle. But my boobs hurt still. They feel lumpy like they are full of milk. It's slightly better on the weekends when she eats from me and no bottles. I rarely feel empty. I feel engorged constantly and like I can't pump all the milk out. The baby doesn't eat long enough for it either.

I had a similar issue. My LC advised me to pump once in the morning so i wouldn't get mastitis and then i slowly reduced that pump over several weeks until I didn't need to anymore. Anyway, I'd recommend checking in with an LC.

I also wonder if something could be tweaked with how you are pumping. If you are that full, you should be getting more than 4 oz in 20 min, so maybe there's something to that?

She needs less breast milk since she’s eating solids and your supply hasn’t adjusted.

I would pump a tiny bit at night to relieve the pressure.

I have to give the nanny milk. She eats three times during my work day, I pump twice and feed her once.

Is something wrong with my pump? I'm using my PISA from my first baby but with all new parts. I did recently get smaller flanges because I measured my nipples and was apparently using ones that are too big.

+1. This is one good/cheap/easy option to try 1st.

FWIW, I used a PISA with my first 3 kids. I ended up getting a new one with insurance after each delivery. The PISA worked great with my 1st and 2nd but didn’t work as well for my 3rd (and it wasn’t the pump - I tried my older one too and had the same results). I got a Spectra S1 for my 4th and it was so much better - emptied me faster and more comfortably. You might want to try a different pump (highly recommend the Spectra).
