What information to have handy for fertility consult?

I am scheduled to see my OB in about a week for a fertility consult. What sort of information do you all suggest I bring to the consult? I have been keeping track of my fertile periods but not exactly saving that data - not recording it online or anything, but I do know my average cycle length and that I usually ovulate on the 14th day of my cycle (+/- a day), with one month being different (later ovulation). What other information should I have availalbe?

First off, I'd see a Reproductive Endocrinologist (RE) about fertility issues instead of your OB. They are the experts and from personal experience you will waste a lot less time by going there first.

In terms of what information to have, they don't seem to be too interested in charts or anything just general cycle length and overall health history. If you are at an RE there is a standard initial workup of day 3 bloodwork and an HSG to check for tube issues.
