Inova health plans for ivf

Does any one have experience with coverage for ivf thru INOVA. I know it covers up to 25k for a lifetime but not really clear on which facilities or How much out of pocket costs will be?

Looking to switch from my fed plan which has no coverage to my husbands employer plan.
