| I just read the other thread where someone in DC and VA was asked to show their card to get a second dose. We got our first dose at our pediatrician's office but they have no plan to give out second doses anytime in the near future. We have an appointment with Montgomery County (they don't seem to do the same open clinics that DC and VA do) but our ped never gave us anything when we got the first dose. |

Agree with all of PP's post. The Maryland Department of Health and Mental Hygiene has a standardized form that all shot recipients are supposed to fill out. You should have gotten the pink copy of this, but if you didn't, the doctor's office (and alternately somewhere in DHMH) should have a copy of this. Agree 100% on the passport of vaccine info. I've got a black one, but it's very handy to have while travelling internationally. I'm glad I've got both the forms and the books showing DC has had both swine flu shots, because we are about to travel to a particularly nervous middle eastern country and I don't relish being aimed at with temperature readers and then being quarantined for no reason.
