When is the dtp generally given?

I have to fill out a form that asks when is my child's last dtp shot. I know my children are up to date on vaccines but don't remember when they got which shot anymore. Is this a required shot, and if so, when is it normally given (that's when we would have gotten it). thanks.

Can't get the info over the phone from the pedi's office and don't have time to go get a shot record before the form is due.

How old is your child, did you follow the regular CDC schedule and do you mean TDap?

I would guess age 4, but it can vary a little.

At my ped's office you can get the 2nd DTaP shot either at age 4 or 5. There were something like 4 vaccines/booster the kid needs to start Kindergarten so people either give 2 at age 4 and 2 at age 5 or just do it all at age 5. So it was probably whenever their well checkup was for the year before Kindergarten.
