Did anyone NOT get a receipt for their shot?

We didn't get a receipt when I took my three to Wilson HS on 10/24. I asked for one, but the DOH rep said we didn't need one.

When I took my three for their second doses on Saturday at a DPR site, the staff wanted to know why I had not received one for their first doses.

I guess that I'll be more insistent next time.

My 18-month old received his first shot at a DC free clinic and we were given a card afterwards that only listed the date on which he should get a booster shot. His regular pediatrician finally got their supply of H1N1 vaccines in so we took him there over the weekend to get his booster. We brought the card with us and the nurse complained that it didn't contain the date he received the shot or information about the vaccination itself such as the lot number and whether it came from a single dose or multi dose vial. We didn't know enough at the time to ask for that information.

we did not get one with anything written on it (we got blank ones with the date) at the wilson clinic on 10-24. My older kids got the mist, but I called the dc flu hotline to find out exactly what shot my youngest got.

Absolutely agreed. Having the manufacturer and lot/batch number(s) is extremely important in the event of a recall or if they find something amiss with a batch. This happened within the past few years with the Hib vaccine, and they were able to specifically ID which batches might have been contaminated. It is unacceptable that they would not issue this to you. When we go to the pediatrician for vaccines, they write down the lot number and manufacturer every time.

We received a card from the Arlington clinic with just the date the mist was administered written on it. The nurse who gave DC the second dose was not happy & I had sign a statement verifying that he was given the mist.

My son also did not get a receipt from the clinic in Arlington.

I think this is something that can happen with public health clinics and 3-hour lines, when they are trying to vaccinate as many people as possible in a short amount of time. Don't know what the excuse is when there is no line, though. Regardless, line or not, certain protocols should be followed--namely, letting you know exactly what was injected or sprayed into your child's body. Sheesh.

I am very annoyed at this also. My DH took DS to get the shot and I asked him to make sure to get all the info from the nurse. He asked, but was told that all he needed was the date of the booster so that is all he got on our card. He didn't know enough to insist. I wish I had thought to call the health department that day.

My son just got his first shot at the Arlington clinic, and the lady almost forgot to give us a receipt. She was so disorganized, that I was more worried about her ability to give my son a shot, that there was no way I would have remembered to ask for one.

Of course, when she remembered, she was about to give my son a shot, so she stopped (yes stopped, waving the needle around) to write it all out, and then returned with the needle. My son earned 2 lollipops for not freaking out : )

We didn't from our peds office (although I hope the wrote down the info). We did from the Reeves center this weekend (and they didn't give us a hard time about not having a card from the first shot).
