| Does anyone have a copy of either SGF's egg freezing contract or their consent form for transferring embryos/eggs to another facility? Thanks! |

| I would call them because these forms change a lot and no one here has an easy way to tell you about theirs or know if it has been updated. |

| I know it may not be exact (although I don't think they change much frequently), but I'm having a oddly difficult time getting SGF to send them to me. So I'd at least like to get a general sense of what I signed. Thanks! |
