United health denial

I am 40, DH, 43. One failed IVF, multiple failed IUIs. All normal blood work. We are in MD, under MD mandate. SG submitted an authorization to United Health for IVF, 2 weeks ago. I called United Health customer service just now and was told that they can see that it was denied for "medical reasons". Have not heard from SG yet. What would be the "medical reasons" for denial? Freaking out here.

I bet they coded something wrong. You get three transfers (for every live birth) in MD.
