HELP! IVF consent forms

Does anyone have copies of the 2 consent forms given to you, from Shady Grove before IVF, 1 was cryopreservation and I dont recall what the 2nd was. Im suppose to turn them in tomorrow at my day 3, and my husband left them at his work. They said it was mandatory that theyre turned in tomorrow.
If you have copies and could scan them and email them to me, Id be very greatfull, and can stop stressing

SG client here: You and your husband are supposed to sign them together in the presence of a SG staff member. So you would not be able to sign them at home and just bring them in anyway. Can he accompany you to the office tomorrow? Then you can get copies from the receptionist and just sign them right there.

You can go to a notary, don't have to do it in the office.
