Two year old house trained dog going inside out of nowhere

Our two year old, adopted shepherd mix, who is completely housebroken has started to go inside the house (pee). It started on one particular rug downstairs, and after one particularly harrowing day where she went upstairs and on the couch we took her to the vet. It, apparently, was not a uti. All tests came back normal. It’s not an everyday thing but is happening enough that my husband is getting super frustrated. For example, she has been out four times this morning but he just found out she peed on her bed! Any ideas?

If there is no evidence of UTI, diabetes, hormone issues, or other medical problems, address it as a training issue and go back to crating her.

thank you. she did it when my husband was home. we do keep her contained in a side room via baby gate when we leave the house.

My female dog who exhibited this had bladder stones. A short term rx diet solved it.
