I’m guessing they work for big companies / entities. I work for a small trade association and get 5 weeks but even taking two weeks off at once is unusual unless it is around Xmas or maybe August. Yes people can sort of cover for a bit but not 100% for long periods.

I get a months vacation every year. My wife likes to go to Europe to see family and we take a month off to go every year. We both have jobs where we arrange for this time and it isn't a problem. We would rather take 1 month off than 1 week here, 1 week there, etc..

Spouse's former U.S. registered company gave a one month vacation as a bonus after 5 years with the company and 6 weeks after 10 years with the company. This could be combined with the company's one week annual shutdown over July 4 and accused vacation. Employees were forced to take the break within a certain timeframe of award so that the company wasn't carrying it on the books as a liability.

I work for the federal government. So, I announced to my boss (two different at different times) that I would be taking the time but gave almost a year notice.

Then we took off 6 weeks the first time and, for various reasons, a month the second.
