Do you ask about turnover when interviewing?

A colleague of mine and I are both looking and he's insistent that one would be nuts not to ask it.

I never have, I wouldn't think I'd get the best answer if I did. They all say what an awesome place to work the job will bring.

Somewhere the interview process, yes, but maybe not first interview.

I do try to understand why they are filling the position, whether it's new or if someone is leaving, etc.

I always ask.... being in the profession of nursing its very key to me to understand the dynamics & promotion/advancing career moves in the department especially in my current nursing specialty. whenever am asked do you have any?? for us it is my 2nd or 3rd question that I ask.

Yes - forget that they are interviewing you - You are interviewing them to see if you want to work there! If it's a toxic environment, you don't want to be there and don't want to waste your time with more interviews!
